Flu Vaccination Consent Forms

Please look out for Flu Vaccination Consent Forms coming home with your child tonight. This is not for children in the Nursery.
This is something that we do in school every year and nothing to do with the current Covid-19 pandemic.
This years immunisation will take place on 23rd October 2020 during the school day.
There is lots of information in the leaflet regarding the nasal sprays.
Please can you fill in the form with a YES or NO answer and return it to school by Friday 18th September.
